1) Original Note Loan Amount

a) Provide a copy of the "Original Hud Note / Deed"

2) Unpaid balance of your current loan?

3) Credit Report Authorization - you & I will do this together.

4) Loan Application

The following info / docs is needed for the FHA Mortgage Insurance / premium

5) FHA case number (this can be found on one of two documents; 1) Note 2) Deed of Trust)

6) SSN, Address, Date of Birth (This gets put into the FHA computer system to determine the FHA upfront premium figures as well as

the amount of the upfront premium that will be refunded).

7) Copy of Drivers License

8) Copy of SS (Social Security) Card
